Wholesale Account Application
Instructions & Information
To be eligible for a wholesale account with V Spa, LLC. you must be in a business that is retailing the products that you are buying, or in a business that uses the products to service the public. An EIN number is required. If you have any questions about whether or not you qualify for an account, please contact us at 818-531-3150 or email wholesale@ilovevspa.com
• To apply for a wholesale account, or to activate your existing wholesale account for online ordering, please fill out and submit the form below. Fields marked with an asterisk (*) are required.
• Please be sure to provide us with a valid email address so that we can notify you once your account has been approved and activated.
• We require that your first wholesale order be for a minimum of $200 less Tax, Shipping & Handling fees. There is no minimum order requirement on subsequent orders.
• Sales tax will be assessed on all orders shipped within California unless we have your California Seller's Permit number on file.
• If you have any additional questions, please contact us.
• You will be able to access wholesale information, such as wholesale (line sheet) pricing, when you receive an email stating that your wholesale account has been ACTIVATED.
• Acceptance of wholesale suppliers is based solely on our discretion, we will only accept wholesaler's whose business vision compliment ours.
